All About Me/Life Trajectory

Charting the LifeCourse is a framework that was developed to help individuals and families of all abilities and at any age or stage of life develop a vision for a good life, think about what they need to know and do, identify how to find or develop supports, and discover what it takes to live the lives they want to live. Individuals and families may focus on their current situation and stage of life but may also find it helpful to look ahead to start thinking about life experiences now that will help move them toward an inclusive, productive life in the future.
An All About Me profile is used by many parents and individuals with IDD to create a brief visual one-pager to help jump start a successful relationship with teachers, therapists, camp counselors and other providers. It is a personal introduction which generally shares the student’s (or family) vision statement, their strengths and support needs, as well as preferences and interests. Much shorter and more accessible than an IEP, it is a person-centered document which can be customized for different activities or settings. By sharing a positively-focussed summary of what is important to and about the individual and how they can best be supported, it helps both the student/participant and provider to put their best foot forward as they begin working together.
